LONDON -- Great granddad may have a thing or two to teach us about safety and investigating accidents. This report from the U.K. publication The Engineer is instructive. It reprints a report from the May, 1912, hearings on the sinking of the Titanic. The reporting on accidents and the public outcry, informed or otherwise, hasn't changed. Neither has the role of the engineer to speak harsh truths even when easier "answers" are available.
